Write your Declaration/Sworn Affidavit letter and cover letter, and take passport photos.
Translate your birth certificate and any dental estimate from the country where you plan to get treatment if it's not in English. Also, get an estimate from a dentist in the USA without dental insurance.
Dentist letters must include the doctor's name, clinic logo, stamp, signature, and date. The more recent, the better.
Remember to add certification to each translated document. The advance parole fee is $575. Always mail copies, not originals. The waiting time is 2-4 months. Get passports for your children/spouse if you haven't yet.

If you're traveling with children without their other parent (or parents), like your sibling, niece, or nephew, and they are underage, make sure to get an International Travel Consent and Acknowledgement (you can download the form here). This helps avoid any problems when you leave or return.
If you are married and have updated your name on your DACA papers, we strongly recommend taking your marriage license or a scanned copy with you.
